How It Works
Steps To Refer
Our nurse-led leadership ensures each referral is reviewed with clinical oversight and professional accountability.
1. Initial
Contact
Call or email our team to discuss the participant’s needs and circumstances.
2. Information Review
We review the participant’s NDIS plan, assessments and relevant documents.
3. Consultation & Planning
Our Registered Nurses discuss needs with participants and key stakeholders.
4. Service Agreement & Commencement
A service agreement is prepared and supports are scheduled to begin.
